Our receptionist will provide you with a registration form and a health questionnaire for completion which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
Please check with our reception staff if you are in our practice area. Once registered you can book an appointment with any of our doctors. We would however encourage patients to consult with the same doctor to ensure continuity of care.

A Patient Guide to GP Services
If you wish to register with the practice you will have to complete both the GMS1 Registration Form and New Patient Medical Questionnaire. These can both be completed by clicking the links below.
Alternatively you may print off the registration and medical questionnaire forms, fill them out and bring them in with you to the practice.
Once we have received these electronically or by hard copy, we will ask you to come to the practice with two forms of ID, one photo ID and one address ID to confirm your identity and sign the forms. You will also be asked to book a New Patient Health Check as part of the registration process.
Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
